This is a mid-term application to the Local Governing Committee for admission to The Three Trees Academies.

  • The person completing this form has legal responsibility for the child. 

  • You have read the guidance notes on the school website.  

  • You complete all sections – failure to do so may delay the processing of this form. 



The form is made available to the Independent Appeal Panel in the case of an appeal against non-admission. In all other respects, the information is treated in confidence.
